Are all areas where workers could be exposed to falls greater than four feet (4 Ft.) guarded by handrails or barriers greater than 39" high

Are ladders inspected before use and taken out of service if damaged

?Are workers who use ladders or man-lifts properly trained

Are employees using the ladders observed maintaining 3-points of contact/control while climbing the ladder

?Are ladders properly stored when not in use

Are workers who access roofs trained to not approach an unprotected roof edge without personal fall protection equipment

Are workers who use personal fall protection equipment (I.e.Harnesses, Lanyards,SRLs, etc.) properly trained in inspection and use of the equipment

?Are Aerial lifts only used by trained personnel

Are Aerial lifts only moved or adjusted horizontally when the aerial lift is in the retracted or grounded position

Are workers operating aerial lift s observed to always remain within the confines of the lift at all times during operation

Are narrow base scaffolds (I.e. rolling scaffolds) in use at the workplace

Are narrow base scaffolds (I.e. rolling scaffolds) inspected before each use and maintained in good operating condition

Are workers on rolling scaffolds observed to lock scaffold wheels to prevent movement while in use

Are workers on rolling scaffolds observed to remain inside guard rails and not climbing on guardrails while working

Are workers on rolling scaffolds observed to not move the scaffold while on the platform (I.e. "scaffold surfing") while in use
